Clinical assessment is (1) collecting clinical data and (2) interpreting what it means.
Many learners find the second part difficult or ellusive and may be confused when asked what their assessment (synthesis) is.
Instead, having examined a patient independently or together, have them complete the following phrase in one sentence:
This patient is in trouble because...

An alternative phrase may be:
Based on the findings of my clincial examination, I think this patient is...
It may be helpful to help by rephrasing the learners summary into more succinct or accurate versions.
If a one sentence summary can be generated, activate clinical reasoning further by asking, 'Based on that, what is our plan?'
